Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts. Insurance copays are required at the time of service. For your convenience, we accept Visa, MasterCard, American Express, and Discover. An additional ten dollar fee will be charged to your account if you are unable to pay your copay at the time of service.

We will submit insurance claims on your behalf. Some insurance companies require a referral from another medical provider before your visit with us. Please check with your insurance company before scheduling your appointment to determine if a referral is needed. Ultimately, it is your responsibility to understand whether your insurance carrier limits you on the doctors you can see or the services you can receive.

Please contact our financial services office or call your insurance carrier should you have questions.
